• Assure administrative and clinical follow-up of the Social and community services’ programs.
• Organize and develop psychosocial and community services.
• Collaborates with the CLSC medical and general directions and community partners.

 Katarina Hayes, S.W., specialist in clinical activities (ext.: 302)

• Assure clinical supervision of Social and community services’ workers.
•  Dispatch all new requests for services (when we receive referrals).
• Assure psychosocial follow-up of clients (all clienteles).

Pamela Bernal, nurse, 0-6 years old program (ext. 328)

• Assure follow-up of 0-6 years old children (to monitor the development of children).
• Plays a major role in the pregnant women follow-ups.
• Ensures optimal vaccination of children.

 Lise Piché, HRO, youth and family services (ext. 321)

• Assure first lines services in person (youth and family program).
• Complete assessments of new requests (intake).
• Collaborates in community events with other services and community organizations.

 Philipp Asante, educator, addiction services worker

• Offers addictions services to clients (all clienteles).
• Refers clients to external resources when needed.
• Collaborates in community events with other services and community organizations.

Roxanne Pien Peastitute, HRO, addiction services worker (ext. 322)

• Assure first lines services in person.
• Offers addictions services to clients (all clienteles).
• Refers clients to external resources when needed.

 Caroline Einish, Community worker (ext. 326)

• Contact person for the Path Finders program (community-based mental health prevention-promotion).
• Plan, organize and lead prevention-promotion activities; work closely with community partners.
• Accompany counselors in their interventions, if needed.

Liliane Souga, nurse (ext. 323)

• Offers nursing services at school.
• Provide community health services.
• Elaborates and participates in health promotion activities and community events in collaboration with other services and community organisations.

 Benjamin Maheux, kinesiologist (ext. 324)

• Offers individual follow-up with clients who want to improve their health.
• Offers activities in the community (walking club, active break,yoga…); acts as a prevention and health promotion agent.
• Works in collaboration with other rehabilitation workers.

   Johanne Proulx, M.A., psychologist

• Offers individual psychological follow-up on TEAMS.
• Works with adult clientele.
• Supports the Social and community services’ workers by elaborating clinical tools.

   Dr. Joseph Beltempo, Ph.D., psychologist

• Complete psychological assessments (all ages with a major on childhood clientele).
• Support Social and community services’ workers in their intervention when needed.

 Dr Lisa Serravalle, Ph.D., psychologist

• Complete psychological and neuropsychological assessments (all ages with a major on childhood clientele).
• Support Social and community services’ workers in their intervention when needed.

  Kelly Desmarais, Public health dental hygienist

• Carry out clinical prevention (sealant application, fluoride varnish treatment), dental health promotion and screening activities; mainly at daycare and school.
• Supports early childhood professionals in screening for dental caries.

   Geneviève Rioux, Nutritionist

• Promote healthy eating and prevent nutrition-related illnesses by hosting activities in the community and culinary workshops to encourage the development of culinary skills.
• Act as a resource for colleagues, organizations and community members.

    Agency workers

• Replace the first line workers during their vacations.
• Assess new requests and offer punctual follow-up (all clienteles).
