Day Center Facilitator-Coordinator - Community Worker (2375) Temporary full-time – 2 years – CSN Local hiring - For resident of Kawawachikamach and Schefferville

The CLSC is located in the Naskapi territory of Kawawachikamach, a region rich in nature and culture. It is responsible for planning, coordinating, organizing, and delivering health and social services to the Naskapi community. Brief description of functions
Person who takes part in identifying the needs of the population and implements various community action programs. They act as a resource person for individuals and groups. Under the supervision of the Head of Medical Services, the incumbent oversees the planning, organization, management, and leadership of the activities of the Naskapi Day Center for seniors experiencing loss of autonomy, while also assisting them with their daily activities at the center.
Specific functions and requirements
SPECIFIC FUNCTIONS
Project Implementation and Deployment
- Develops tools and schedules required for the implementation of the Day Center;
- Sets up and prepares the Day Center premises;
- Designs and plans, physical stimulation, cognitive stimulation, recreational, socialization, and intergenerational activities for Day Center users, taking into account their abilities, needs, interests, and culture; Management
- Submits budget requests necessary for the proper functioning of the Day Center (e.g., meals, furniture, activity materials, etc.);
- Communicates with caregivers to facilitate the reception and integration of the users;
- Participates in the reception and integration of volunteers, as needed;
- Participates in relevant training related to the role;
- Writes observation notes, recommendations, and an annual report;
- Collects and prepares statistics;
- Acts as an active member of the advisory committee for the Naskapi Day Center and seniors’ residence;
Daily Operations
- Welcomes users and ensures their well-being;
- Coordinates and leads activities for the users;
- Assists seniors in preparing for and participating in group activities, recreational activities, and personal care (e.g., hygiene, dressing, mobility);
- Ensures the management of Day Center equipment;
- Creates a relaxed atmosphere and de-escalates situations that may lead to tension;
- Plans meals, snacks, and transportation in collaboration with community partners;
- In cooperation with the personal support worker, participates in coordinating the meal period;
- Support for translation;
Multidisciplinary Tasks
- Works closely with various partners (community-based organizations, etc.);
- Serves as the contact person for the Naskapi Day Center;
- In collaboration with the CLSC Naskapi home care social worker and nurse, contributes to the selection, referral, and assessment of the users;
- Collaborates with various partners to enhance the programming of activities (e.g., artists, therapists, health professionals);
- Participates in meetings with the multidisciplinary social and community services team, as well as the home care team; Performs any other related duties as assigned by the manager, consistent with the employee’s role and competencies.
REQUIREMENTS
- Fluency in oral Naskapi;
- Fluency in oral and written English;
- Knowledge of Naskapi traditions, beliefs and customs;
- Must hold a college diploma in a relevant discipline. A combination of a high school diploma with relevant experience might be considered;
- Experience working with seniors is considered an asset;
- Genuine care for seniors and a desire to contribute to their well-being;
- Proficiency in Word and Outlook;
- Experience in project management is considered an asset;
- Safe Patient Handling and Mobility Program (SPHMP) training will be considered an asset;
- Experience in group facilitation, helping relationships and/or issues faced by individuals experiencing a loss of autonomy;
- Demonstrates a positive attitude and a strong sense of responsibility and organization;
- Ability to work both independently and as part of a team;
- Reliable, dynamic, good sense of initiative, autonomous, creative;
- Possesses a valid driver’s license.
Priority will be given to the beneficiaries of the Northeastern Quebec Agreement
Working conditions
- Hourly rate depending on experience (between 27,71$/h and 39,88$/h);
- Day shift, 35h/week, Monday to Friday;
- Will be required to occasionally work on evenings and weekends for special activities;
- Responsibility premium of 10%
- Isolation premium of $227,69/week of 35 hours (without dependent) or $401,36/week of 35 hours (with dependent); paid each pay period according to hours worked;
- Retention premium starting at $1,513 annually for an employee with less than one year of continuous service at the Naskapi CLSC;
- Cargo premium $83.75/week of 35 hours (adult and child 12 years and older), $41.88/week of 35 hours (child under 12);
- Group insurance;
- Pension plan (RREGOP).
