Brief description of functions
A person who, in addition to their duties as a nurse, conducts patient assessments—due to the absence of a physician permanently assigned to an outpost or dispensary—that enable the physician to make a remote diagnosis and determine the appropriate interventions. They carry out activities and interventions that are
generally reserved for physicians in other work settings.
Must be a member of the Ordre des infirmières et infirmiers du Québec (OIIQ).
Provides emergency on-call coverage for the community (on a voluntary, on-call basis), alternating with the onsite care team, as a first or second responder.
Specific functions and requirements
Responsibilities of the Home Care Nurse:
- Provides nursing care in patients’ homes in order to improve their quality of life through a holistic, community-based, and multidisciplinary approach;
- Performs duties related to home care services for individuals experiencing loss of autonomy, convalescence, etc.;
- Assesses the client’s health status and develops and implements the nursing care and treatment plan;
- Provides nursing and medical care and treatments with the goal of maintaining health, restoring it, and preventing illness;
- Plans health services related to health promotion, prevention, treatment, and rehabilitation;
- Identifies the health needs of individuals experiencing permanent or temporary loss of autonomy using the Multi-Client Assessment Tool (OEMC);
- Acts as team leader and supervises a home support team:
- Collaborates in the establishment and proper functioning of the Naskapi Elders’ Day Center:
- Conducts client assessments (OEMC);
- Prepares required documentation;
- Participates in the admissions committee;
- Facilitates certain workshops;
- Provides support to Day Center staff;
- Performs any other duties as required by their supervisor.
REQUIREMENTS
- Must be a member of the Ordre des infirmières et infirmiers du Québec (OIIQ).
- Three (3) years of relevant experience;
- One (1) year of experience working with seniors, in community health, or in home care;
- Extended Role training; must have authorization for collective prescriptions related to their duties or have successfully completed Extended Role training. A candidate who meets the requirements but does not yet have Extended Role training may be considered and trained at the employer’s expense;
- Foot care training (an asset);
- Experience working with First Nations or in a remote region (an asset);
- Proficiency in spoken and written English;
- Excellent interpersonal and teamwork skills;
- Interest in sharing knowledge and experience with colleagues;
- Strong organizational skills, creativity, initiative, and autonomy;
- Demonstrates openness to cultural differences, as well as interest in and empathy toward others;
- Good ability to adapt to a new environment;
- Valid driver’s license.
Working conditions
- Hourly rate in accordance with the Nomenclature of Job Titles, Descriptions, Rates and Salary Scales of the Health and Social Services Network, based on the job title associated with the candidate’s diploma;
- Day shift, 37.5 hours per week, Monday to Friday;
- On-call shift (voluntary basis);
- Isolation premium of $227.69 per 37.5-hour week (without dependents) or $401.36 per 37.5-hour week (with dependents);
- Cargo premium of $83.75 per 37.5-hour week (adult and child aged 12 and over), $41.88 per 37.5-hour week (child under 12);
- Retention premium of $287.47 per 37.5-hour week;
- Dispensary premium of $222.00 per 37.5-hour week for 1911 – Clinical Nurse only (for 2491 – Dispensary Nurse, the dispensary premium is included in the hourly rate);
- Furnished housing provided, all utilities included (high-speed fibre optic Internet), if located more than 50 km from your place of residence;
- Moving and storage expenses reimbursed by the CLSC;
- Travel day and travel expenses paid;
- Four (4) annual outings for full-time employees with “without dependent” status: the full-time work schedule averages 8–9 weeks worked followed by 4 weeks off;
- Three (3) annual outings for full-time employees with “with dependent(s)” status: the full-time work schedule averages 12–13 weeks worked followed by 4 to 5 weeks off;
- Group insurance plan.
- Possibility of obtaining a Northern leave if currently working within the health and social services network;
