Brief description of functions
Person who carries out design, evaluation, orientation, consultation, updating and analysis activities in one or more social programs. He carries out interventions at the psychosocial level in an interdisciplinary philosophy and collaborates in the different stages of programming the activities of the service to which he is assigned and in the evaluation of quality, at the request of his immediate superior. He participates in promotion and prevention activities.
Specific functions and requirements
Under the authority of the head of social and community services, his main areas of intervention are:
- Provides psychosocial interventions to adults, seniors, and individuals experiencing loss of autonomy by delivering individual, couple, family, and group services in accordance with client needs and the program's service offerings;
- Conducts psychosocial assessments, develops intervention plans, and provides support, guidance, information, and referral services;
- Intervenes in a variety of psychosocial and crisis situations, including those related to mental health, addictions, domestic violence, and suicide risk;
- Collaborates with internal and external partners to ensure a comprehensive approach to client care;
- Plans and participates in prevention, health promotion, and community development activities, particularly those involving seniors;
- Maintains client records and prepares statistical reports;
- Contributes to the assessment, follow-up, and coordination of services by participating in home visits, case discussions, and clinical and interdisciplinary meetings;
- Adapts interventions to Naskapi culture and values;
- Participates in the psychosocial on-call rotation during evenings and weekends, according to the established schedule;
- Performs any other related duties as required by their immediate supervisor.
REQUIREMENTS
- Must hold, depending on the required field of activity, a bachelor's degree in human sciences or social sciences such as social work, psychoeducation, criminology or psychology. In the absence of a Human Relations Officer who meets all the requirements of the position, a candidate with a college degree in an appropriate discipline (e.g. social work technician or special education) may be considered. If a technician candidate is selected, the applicable salary range will be that which corresponds to his/her level of education;
- Member of a professional order (a major asset);
- Two (2) years of relevant experience with youth-family clients (an asset);
- Experience working in a remote region (an asset);
- Fluency in oral and written English;
- Excellent interpersonal and teamwork skills;
- Interest in sharing knowledge and experience with colleagues;
- Demonstrates initiative, organization and autonomy;
- Demonstrates open-mindedness in the face of cultural difference, interest and empathy towards others;
- Good ability to adapt to a new environment;
- Valid driving license.
Working conditions
• Hourly rate according to the Nomenclature des titres d’emploi, des libellés, des taux et des échelles de salaire du réseau de la santé et des services sociaux, based on the job title related to the candidate’s diploma;
• Day shift, schedule of 35h/week, from Monday to Friday;
• Availability shift;
• Possibility of job sharing;
• Isolation premium of $233,38/week of 35 hours (without dependent) or $411,84/week of 35 hours (with dependent);
• Cargo premium $83,75/week (adult and child 12 years and older), $41,88/week (child under 12);
• Retention premium starting at $6,050 annually for an employee with less than one year of continuous service at the Naskapi CLSC;
• Furnished accommodation provided, everything is included (high-speed fiber optic Internet), if located more than 50 km from your place of residence;
• Moving and storage costs reimbursed by the CLSC;
• Transportation day and travel expenses paid;
• 4 annual outings for full-time employees with a “no dependent” status: The full-time work schedule averages 8-9 weeks worked and 4 weeks off;
• 3 annual outings for a “with dependent(s)” status: The full-time work schedule averages 12-13 weeks worked and 4-5 weeks off;
• Possibility to obtain a Nordic leave if you are working in the Health System;
